Booth Application

Sunday September 18th 2011
Parade 1:00 pm to 2:00 p.m.
Street celebration from 12:00 p.m. to 7:00 p.m.

Instructions:

• Read and fill out the application
• Please make your check out to Golatino!/Urban Ventures. Please make another $50.00 check for the cleaning deposit, this deposit will be returned to you at the end of the event (if and when you follow the cleaning rules).
• You may also send this application by mail by using the PDF link.  Please send this application to the official address of Urban Ventures or take it to the offices from 11:00am to 4:00pm, Monday through Thursday.
• Once we receive your application, it will be processed and if accepted we will send you your receipt for payment.
• If the products and/or services that you offer do not support the mission of the event, your application will be denied and we will send your money back within 5 days after we receive it.

The success of the event is based on the vendors and administrator at the event. please show up with a positive attitude and let's make the challenges an opportunity for unity.
